Grace Place

Dinner Wednesday evenings at 5-6:30pm except some Holidays

A volunteer staff will help to serve fresh, healthy meals to those who are in need. Household and personal items will be offered at no charge when available, We will strive to help connect people with the resources they need, and to offer hope and encouragement to those who are downcast. Volunteer groups, organizations, or individuals are always welcome and encouraged. What we share is common, work on together, cultivate for the benefit of others, pulling together, pulls together. That’s what community means; that’s why it’s a “Community Table”, and there is always room for one more at the table.

Healthy Opportunities Pilot

The Healthy Opportunities Pilot is the nation’s first comprehensive program to test evidence-based, nonmedical interventions that are designed to improve the health of Medicaid beneficiaries while reducing healthcare costs. This innovative program is being piloted in three regions across the state, including right here in Western North Carolina. HOP provides support for social needs related to food, housing, transportation and interpersonal safety. Detailed information about the pilot can be found on the NCDHHS website (ncdhhs.gov/healthy-opportunities-pilots).

To be eligible for pilot services, individuals must be enrolled in a NC Medicaid managed care health plan, live in one of the state’s three pilot regions, which includes NC’s 18 westernmost counties, have a qualifying physical or behavioral health condition + a qualifying social need or risk factor.
